Then download and print the Order Form, fill it out completely and mail or fax it, along with your prescription, to the pharmacy.

If you are unable to print this PDF form, please email us at forms@northcountryrx.com with your name and and complete mailing address and we will be happy to send you the Order Form by post mail. You should receive it within three to four days.
